The research on high-load strengthening for plantar fasciitis is clear. What's less clear, for most people, is exactly how to put it into practice. How do you start? How do you progress? What does a full 12-week program actually look like?
This is a practical guide, based on the Rathleff et al. protocol and clinical experience, to help you structure your program from day one to week twelve. Always work with your health professional to adapt this to your individual needs and capacity.
The Principles Behind the Program
Progressive overload: The load on the plantar fascia increases over time. The tissue adapts to the demands placed on it, so those demands need to gradually increase for adaptation to continue.
Consistency over intensity: Daily or near-daily training at a manageable load outperforms occasional high-effort sessions. The tissue adapts through regular, repeated stimulus, not through occasional hard effort.
Toe extension position: Every heel raise in this program is performed with the toes extended. This engages the windlass mechanism, placing targeted load through the plantar fascia along its full length. Without toe extension, the exercise loses much of its specificity.
Pain monitoring: Some discomfort during the exercise is normal and expected. Pain that increases beyond a 4 out of 10, or that is significantly worse the following morning, is a signal to reduce load or rest. Stay within tolerable limits and progress will come.
What You'll Need
- A Fasciitis Fighter or Fasciitis Fighter ROUND 2 (for consistent toe extension position)
- A sturdy surface for balance support
- A backpack for progressive loading (Weeks 5–12)
Phase 1: Foundation (Weeks 1–4)
Goal: Establish the movement pattern and build initial load tolerance
Exercise: Two-leg heel raises on the Fasciitis Fighter ROUND 2 (or single-leg on the original)
Tempo: 3 seconds up, 2 second hold at the top, 3 seconds down
Reps: Start with 3 sets of 8–10 reps
Load: Bodyweight only
Frequency: Once daily, with rest days as needed
The priority in Phase 1 is technique, getting comfortable with the toe extension position, controlling the tempo, and finding a load that produces mild discomfort without significant pain.
Phase 2: Loading (Weeks 5–8)
Goal: Progressively increase load through the plantar fascia
Exercise: Transition to single-leg heel raises (ROUND 2 users) or continue single-leg with added load
Tempo: 3 seconds up, 2 second hold, 3 seconds down
Reps: 3–4 sets of 10–12 reps
Load: Begin adding weight via backpack, start with 5kg, increase by 2–5kg every 1–2 weeks as tolerated
Frequency: Once daily
This phase is where the most meaningful tissue adaptation occurs. The progressive addition of weight is what drives the stimulus for remodelling and strength development.
Phase 3: Consolidation (Weeks 9–12)
Goal: Build strength at higher loads; begin reducing frequency
Exercise: Single-leg heel raises with progressive loading
Tempo: 3 seconds up, 2 second hold, 3 seconds down
Reps: 4 sets of 12–15 reps
Load: Continue to increase as tolerated, many people reach 15–25kg of added load by Week 12
Frequency: Once daily, or every second day as strength improves
Complementary Exercises
Calf and ankle mobility: 2–3 minutes of calf stretching daily. A slant board makes this significantly more effective than floor stretching alone.
Intrinsic foot strengthening: Toe spreading, short foot exercises, and resistance band toe exercises (3 x 10 reps, 3–4 times per week). The Toe Strong bands are purpose-built for this.
Arch activation: Short foot bridge exercises on the Foot Bridge, 2–3 times per week from Week 4.
After 12 Weeks
The research shows continued improvement beyond the 12-week mark for many people. Once you've completed the program, the goal is to maintain the strength you've built, through regular loading (2–3 times per week) and a gradual return to normal activities.
A Final Note
This program works. The research supports it, and clinical experience confirms it. But it requires consistency, patience, and a willingness to progress gradually. Done correctly, 12 weeks of this protocol can achieve more than months or years of passive management.
